HEALTH SCREENING PACKAGES SOLD AND SERVICES RENDERED ARE NON-RETURNABLE AND REFUNDABLE, ALL LEGAL RESPONSIBILITIES ARE HEREBY EXCLUDED.

Lab request form bearing Original Healthworks Corporate Wellness stamping should be presented for redemption. Only original Lab request form is valid for used ONCE, and must be presented at the following Innoquest labs - Royal Square @ 101 Irrawaddy Road, #07-02, Singapore 329565 (Tel: 6734 0778), Paragon Medical Centre @ 290 Orchard Road #17-07 (Tel: 6737 2788), Singapore 238859 or Camden Medical Centre @ 1 Orchard Boulevard, #09-06/07, Singapore 159357 (Tel: 6333 6640).

HealthWorks' health screening package is valid for a period of one (1) year up to the expiry date or from date of issue as stated in the request form.

HealthWorks Corporate Wellness reserves the rights to add, amend or withdraw any terms and conditions without prior notice.

Effect of law

ALL LEGAL RESPONSIBILITIES ARE HEREBY EXCLUDED.

We will keep customer's information as long as he/she remains as our customer. Or a maximum of five years if our service(s) is not engaged by customers for five consecutive years.
